FBI 258 Fingerprint Card, standard applicant issue.
If you do not have one,  we do carry them in stock, no additional charge. 
Some states and countries outside the US, may have their own card or form. 
Be sure that you provide the necessary paperwork required.  
When mailing your card, use a large manilla envelope, with some stiff cardboard to keep them straight. 
Extra postage may be necessary.

Instructions for filling out your fingerprint cards.
Please note before you come to your  fingerprint appointment, DO NOT SIGN THEM,
The name on the card must match the name on our photo id or passport.
Cards are to be filled in with  black ink, and do not fold them!

NAM= Name: Last Name, First Name and  Full Middle Name
Suffixes denoting seniority (Jr.,  Sr., Should follow the first or middle name

DOB= Date of Birth: in month, day, year format (i.e. MM/DD/YYYY).

CTZ: Citizenship
Enter U.S. if your citizen of the United States
Otherwise list the appropriate country.
Yes or No is not an answer!

POB= Place of Birth
Inside the US: city and 2 digit state    Seattle WA
Outside the US:   Country

Hair: Use the following codes
Black=BLK
Brown=BRO
Bald=BAL
Red=RED
Blonde=BLN
Sandy=SDY
White=WHI
Gray=GRY
Unknown=XXX

EYES: Use the following codes

ORI: This is an assigned reporting number and needs to be included. If you do not have one, the
agency being reported to, will need to supply one.  Often listed online.

Signature on the card: Must Match the name at the top of the Card, and must be done in ink. No pencil or colored pens.
Signature of Person Doing Fingerprinted: Must be signed by the fingerprint technician
Residence of Address: Is your home address, not your mail box POR
Reason Fingerprinted: Should be supplied by agency or organization
Address of Employer:  List the name and address of your potential employer

It is important to fill out the cards with all of the information that is asked.  Not filling out your cards could mean a rejection of your cards or delay in the processing.
Some questions that you do not have an answer for such as your FBI or Military number. Leave it blank, that is the exception.

Unless specifically required by the agency requesting your fingerprints, we do not mail them.
The cards are given back into your custody. We do not retain any personal information.
